About Us

The Diabetes Coalition of Santa Clara County has been in existence since March 2002. Earlier that year, the grants staff of the Health Trust had noted an increase in the number and urgency of requests for support for diabetes programs from a wide range of providers and organizations throughout the region. The proposals all noted critical gaps in diabetes prevention, early detection, access to care, and case management, yet neither the proposals not the proposed programs were linked or strategically coordinated. In an effort to stimulate program coordination and strategic thinking about diabetes in Santa Clara County, The Health Trust, along with the Community Health Partnership and the Diabetes Society of Santa Clara Valley, convened a meeting of 25 different provider groups, non-profit organizations, researchers, health plans and local hospitals, nurse parish programs, and local foundations. Diabetes Coalition Sponsors "Diabetes Care: How Can We Do It Better?"

The Diabetes Coalition of Santa Clara County and the Countywide Chronic Care Coalition invite you to help change Diabetes Care in Santa Clara County. Whether you are personally affected by diabetes, know someone who has diabetes, or a health care professional come join us for “Diabetes Care: How can we do it better?”

If you are personally affected by diabetes you will have the chance to tell us what you would like improved in our health care system. If you know someone who is affected with diabetes, learn what you can do to help that person improve his or her health. If you are a health care professional, learn about new models of health care management. The two hours you will spend with us will help to improve health care in our county.

Admission is free. Snacks will be provided.
